The Goodwin Log - Chapter I

"...I am a consultant anaesthetist working in a large teaching hospital in Wales, UK. My special interests include anaesthesia for renal transplant, orthopaedics, ENT and major gynaecological oncology.

I am not sure what I will find in Tanzania. I know that it will be very different ! I expect that the equipment will be very basic with draw-over anaesthesia; a lot of regional techniques, and of course ketamine..."

"Summery: The TruView EVO2™ laryngoscope was compared with the traditional Macintosh laryngoscope in 200 patients who required tracheal intubation for elective surgery. Mallampati score determined prior to laryngoscopy was significantly related to the view of the glottis during laryngoscopy for both laryngoscopes. The view of the larynx was better with the TruView EVO2™ laryngoscope than with the Macintosh laryngoscope in patients with a Cormack and Lehane grade greater than 1 (p<0.01). The mean time to intubate was significantly shorter with the Macintosh laryngoscope (34s) than with the TruView EVO2™ laryngoscope (51s) (p<0.01)..."

"Summery: The Truview blade facilitates a view of the vocal cords by indirect laryngoscopy. We prospectively compared the view obtained at laryngoscopy and intubating conditions of Truview (Group1) or Macintosh (Group2) blades in 170 patients who were scheduled to undergo general anaesthesia. We studied pre-operative airway evaluation, laryngoscopic view, duration of intubation, maximal force applied during intubation, anaesthetist’s estimation of intubation effort on a 1–3 scale, bleeding, teeth and soft tissue damage, and postoperative stridor and hoarseness..."

Laryngoscopy — its past and future
by Richard M. Cooper, BSc MSc MD FRCPC

"MAN’S assumption of an upright posture, coupled with our tendency to live in social groups has resulted in some bad habits - simultaneous eating and talking. This has necessitated exclusion of the larynx from the line of sight connecting the mouth to the esophagus. While this does make eating safer and more interesting, it has complicated the task for airway managers..."

    • Reduces difficult by atleast one grade, making difficult cases easier to intubate.

    • Continuous oxygen flow at 10 litre per minute extends the window of intubation ;cleans away secretions & prevents fogging.
