The Goodwin Log - Tanzania 


 We are off to Tanzania…. How it started …

Six years ago, my husband, a dermatologist, went on a study trip to Tanzania to look at skin diseases in Africa. He came back filled with enthusiasm for the country and people and determined that we should go back there, as a family, to work there for a period of time. While I was equally excited, the thought of dragging two very young children to live in a foreign African country was daunting. Now we have three children; aged 10, 8 and 5. If we do not go now, then there will never be a right time so we have decided to take the plunge for six months.

 

I am a consultant anaesthetist working in a large teaching hospital in Wales, UK. My special interests include anaesthesia for renal transplant, orthopaedics, ENT and major gynaecological oncology.
 
I am not sure what I will find in Tanzania. I know that it will be very different ! I expect that the equipment will be very basic with draw-over anaesthesia; a lot of regional techniques, and of course ketamine

Truphatek have kindly given me some airway equipment to take out with me.

While I expect that I will get most use out of the standard laryngoscope set, it will be very interesting to see how the Truview works in a difficult environment. I am hoping that its simplicity could make it a good tool for difficult intubations. I certainly do not expect to have access to flexible fibre-optic scopes. If I can prevent dust and dirt getting into the camera and keep it working then hopefully I can make the Truview into a very useful teaching device..
 
 
 
 

We are leaving tomorrow - our bags are nearly packed and we have our visas. I have spent a fortune on anti-malarial treatments and my poor children have been subjected to five different immunisations.

We are really looking forward to catching our plane.

Laryngoscopy — its past and future

This has necessitated exclusion of the larynx from the line of sight connecting the mouth to the esophagus. While this does make eating safer and more interesting, it has complicated the task for airway managers..."

 

  • Reduces difficult by atleast one grade, making difficult cases easier to intubate.

 

  • Continuous oxygen flow at 10 litre per minute extends the window of intubation ;cleans away secretions & prevents fogging.
